What is a Referral Code?
A Referral Code is a unique number (1-number) you receive when your Support at Home is assigned to you.
Important distinction:
● SAH Approval is when you’re assessed and approved for a certain care level (1 to 8).
● SAH Assignment is when funding is made available for you — this is when you receive your Referral Code.
Why is a Referral Code important?
You’ll need to give your Referral Code to your chosen Home Care Provider. It allows them to:
● Trigger your government funding
● View your assessment details
● Understand your care needs before accepting you as a client
We ask for your Referral Code before taking you on because we want to make sure we’re the right fit for you, and that we can meet your care needs properly.Note: A provider can view your documents with the code, but they can’t activate it (or accept you as a client) unless you agree.
How to find your Referral Code:
If you’re new to Home Care:
Your red Referral Code is on the letter you received from My Aged Care when your package was assigned. Can’t find it? Just call My Aged Care on 1800 200 422 and ask for it.
If you’re switching Providers:
You’ll need to reactivate your code, as your previous Provider would have already used it. Reactivating is quick — call My Aged Care on 1800 200 422 and ask them to reactivate your Referral Code.
